Job Description

Primary Function

Through effective relationships, the ED Patient Navigator helps patients/family recognize patterns of exacerbation of chronic conditions/illnesses and resource use. Helps the patient define goals for health management and quality of life. The ED Patient Navigator major functions include:

*Identification of at-risk patients

* Coordination of patient/family education related to self-care

*Coordination of community and agency resources to meet the patient needs

*Regular assessment, monitoring, and evaluation of care provided

*Provision of emotional support, counseling, and necessary referrals

*Performs liaison functions and patient advocacy with physicians, nurses, and community.

The ED Patient Navigator can serve as a bridging tool when patient is no longer eligible for home health, through helping patient/family identify and coordinate other resources. Assumes responsibility for case managed patients through 24-hour/day availability. Prepares regular reports on the effectiveness of the program.

Knowledge and Experience Required:

A. Education:

Master of Science in Nursing or Master in Social Work (strongly preferred)

Bachelor of Science in Nursing or Bachelor of Social Work required

B. Licenses and/or Certifications Required:

Board of Nurse Examiner (Texas) license or

Licensing Board for Social Workers

C. Related Work Experience:

5 years of health care experience

Knowledgeable of multiple case management applications

Proficient with clinical issues for populations served

Must have computer knowledge and competency with office suite (e.g. Excel,

Word, PowerPoint)
