Within a multidisciplinary team, manage all aspects of care for internal medicine patients, including many patients with chronic illnesses such as diabetes, hypertension, congestive heart failure, hyperlipidemia, asthma and other conditions.
Obtain detailed history and perform physical examinations. Establish medical diagnosis(es) and determine a plan of care for preventative care services, short-term health problems, chronic stable health problems, and/or exacerbation of chronic health problems.
Order and interpret diagnostic tests, including laboratory and radiology procedures within practice scope. Seek licensed physician interpretation for results when necessary.
Provide education to patients on a variety of health topics in order to improve patients’ compliance with medical recommendations and health status. Utilize a team approach to providing health education; and collaborates and communicates with team members
Maintain all clinical information in the electronic medical record system, including health status, treatments and plans.
Refer patients to appropriate licensed physicians, clinics, or other health providers for the purpose of specialized care outside of practice scope.
Contributes to and participates in continuous quality improvement process to measure improvement and opportunities for improvement.
