Manager, Patient Care Coordination Services

Responsible for assisting the Director, Care Management in overall operations of Patient Care Coordination Services (PCCS) Department functions through daily and long-term oversight of department activities at Wayne UNC Health Care. Provides management direction to staff members. Assists Director in ensuring that the department achieves its goals and objectives by engaging internal and external stakeholders in clinical care coordination and outcomes management processes. Accountable for departmenal personnel management, fiscal management, communication, customer service, goals/outcomes management, and quality/process improvement initiatives to ensure high level of quality consistent with organizational standards.

Education: Bachelor's degree from an accredited nursing school (BSN preferred), MSW, BSW, or other professional degree with appropriate clinical and/or project management experience. Certification in case management preferred. Will consider relevant combination of education and experience in lieu of advanced degree.

Experience: Five years experience in care management required, i.e. case management, utilization management, discharge planning, outcomes management, project management and/or care coordination preferred; two years healthcare management experience required. Prefer two years demonstrated leadership experience. Knowledge of TJC, federal, and state standards and health care operations required.

Skills: Strong communication, software, project management, leadership, organizational, computer, and analytical skills. Ability to build and maintain collaborative relationships at all levels. Critical thinking and problem resolution skills required.
