The Clinical Nurse B, Home Health Care, is a field nurse that provides age-specific and developmentally appropriate quality patient care in the home environment following a patient-centered care model of practice. Utilizes discretion and independent judgment and performs the nursing process with minimal supervision. Functions in a skills mix environment and directly supervises and delegates tasks to licensed and unlicensed multidisciplinary staff. Coordinates patient and staff assignments, patient care, referrals and ensures that patient’s needs are continually assessed, reassessed and addressed through the development, implementation and updating of the patient’s individualized plan of care. Applies theoretical concepts to clinical practice and continually expands knowledge base and clinical skills with a focus in home health care. Facilitates the empowerment of the nursing profession and embraces new knowledge, innovations and improvements in practice to enhance the patient experience and clinical outcomes. Utilizes interpersonal skills to maximize excellence in customer service, safe patient care, and professionalism. Contributes to a work environment of caring and cooperation among a culturally diverse workforce and patient population. May be required to participate in on-call rotation based on department business needs. Performs other duties as needed.
Associate’s Degree in Nursing required. Bachelor of Science Degree in Nursing preferred. Minimum one year nursing experience required.
Must be able to drive and have reliable transportation and a good driving record. Basic knowledge of state and federal home healthcare regulations, finance, performance improvement, quality, patient safety, evidenced-based clinical practice standards, and legal aspects of care required. Knowledge of pediatric and adult learning principles and problem-solving methodologies. Exhibits professional behavior and appearance at all times. Able to speak, read and write legibly in English (and Spanish preferred) with professional quality; use computer, printer, and software programs necessary to the position, e.g., Word, Excel, Outlook, PowerPoint, electronic medical record, electronic event reporting program; relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction; think critically; use sound judgment based on factual information and clinical knowledge; work independently with minimal supervision; meet deadlines; perform basic math and statistical functions; manage multiple assignments effectively; organize and prioritize workload; problem solve in a proactive and creative manner; recall information with accuracy; pay close attention to detail; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place; see adequately to read computer screens, medical records, and written documents necessary to position.
Active California Registered Nurse Licensure required. Basic Life Support (BLS) certification from the American Heart Association required. Valid Driver’s License and automobile insurance required at time of hire.
