Req ID: 25939 Shift: Day Employment Status: AF=Active-Regular-Full Time Job Summary The Nurse Navigator will practice within their scope of practice and under the guidelines of the Division of Orthopaedic Surgery. In collaboration with the CTIS Physician, Director, Assistant Director, Program Manager and Advance Practice Providers. The Nurse Navigator will provider indirect clinical care and follow up to patients referred for complex chest and spine abnormalities. Responsibilities include coordination of care, indirect patient contact, review of medical record, oversee complex scheduling and ombudsman for CTIS patients and families. The CTIS Navigator Nurse will assist and support CTIS Program Manager with program development, program management, patient family education, care coordination and professional development. Job Responsibilities Expedite and facilitate communication for patient consultation, referrals, second opinions, and transfers with focus on pediatric spinal deformities. Coordinate care with referring clinicians and referral patients to simplify access, improve communication and enhance the patient/ family experience. Coordinate a plan of care for spine deformity patients and serve as primary contact for referring physicians Coordinates initial visit to CTIS orthopedist for local families with subsequent return visit for full evaluation by CTIS team, and coordinates global evaluation visit for out of state/international families for multi-specialty clinical evaluation and testing Determines evaluation needs of new CTIS patients and projects timeline for completion. Places orders for consults, imaging and testing in EPIC). Coordinate complex scheduling activities of CTIS Clinical/Surgical Coordinator,including orthopedic evaluation, pediatric general surgery clinical evaluation, pediatric pulmonary evaluations, genetic evaluations, pediatric neurosurgery evaluations, and neurology evaluations when needed, infant/standard spirometry pulmonary function studies/sedation, radiology imaging appointments, including radiographs, scans, dynamic lung MRIs under sedation/ general anesthesia, spinal cord MRIs under general anesthesia/sedation, other tests as necessary. Ensures completion of tests during projected evaluation window, and timely scheduling of surgery. Reviews test results and letters from consultation visits. Follows up on abnormal results or additional patient needs. Provide the patient and the referring physician with updates and pertinent medical information. Act as point person for referral base (patient/ families/ PCPs) . Coordinate transport and direct admissions. Participate in ambulatory appointments to assure ongoing care coordination. Utilizes advanced assessment skills to assist with management of CTIS patients with complex chest/ spine anomalies with pulmonary disease. Contribute to the ongoing educational and professional development of staff, students and residents in the area of TIS diagnosis, treatment and surgery. Required Licenses, Certifications, Registrations Licensure as an RN in the Commonwealth of Pennsylvania required; NJ if applicable. Required Education and Experience Required Education: Graduation from an ACEN and/or CCNE school of nursing or college. Required Experience: Five (5) years of experience with surgical populations in both inpatient and ambulatory settings. Preferred Education, Experience & Cert/Lic Preferred Experience: Experience working with patients/families with complex surgical needs
