Job Summary
Coordinates a interdisciplinary plan of care or program to meet the needs of a specific patient population. Functions as a clinical, patient, and community resource, educator, and consultant.Prisma Health Heart and Vascular Institute
Prisma Health Heart and Vascular Institute is South Carolina’s first comprehensive, freestanding hospital dedicated solely to the prevention, diagnosis, and treatment of cardiovascular disease. Open to all doctors and patients, our continuum of care begins with disease prevention and management and extends to emergency services and procedures, with diagnosis, surgery, and rehabilitation, all performed in beautiful surroundings of our hospital.
Essential Functions

Participates in patient care activities for the patient population(s) served including direct patient care, indirect patient care, patient rounds, consultation, multidisciplinary care planning, and evaluation.
Collaborates with other health team members to develop, implement, and evaluate quality programs to meet the needs of population(s) served in a cost-effective manner.
Develops, provides, and evaluates educational programs to meet the identified needs of the population(s) served including patients, families, staff, and the community.
Monitors and ensures accuracy of documentation on patient records. Maintains all required reports, records, and statistics related to program.
Monitors the quality of care delivered to the population(s) served on a continual basis; identifies quality issues and integrates process improvement (P.I.) into practice.
May participate in research/investigative studies when indicated.

Minimum Requirements
Education - Associate degree in Nursing. Bachelor's degree in Nursing preferred.
Experience - Three (3) years of nursing experience.
