Summary:
The Care Management Clinical Educator plays a crucial role in the development and implementation of education programs within the care management department. This position involves working closely with healthcare professionals to ensure that best practices are incorporated into their daily practices. The primary responsibilities of the Care Management Clinical Educator include:
Responsibilities:
1. Develop and Implement Education Programs: The Clinical Educator is responsible for designing and implementing educational programs that enhance the knowledge and skills of care management staff. This involves staying up to date with the latest evidence-based practices and incorporating them into the curriculum.
2. Conduct Trainings: The Clinical Educator will deliver engaging and informative training sessions to care management staff. These trainings may cover a range of topics, including patient assessment, care planning, documentation, and communication skills. The educator must be able to effectively convey complex information in a clear and understandable manner.
3. Coach/Mentor: The Clinical Educator will provide one-on-one coaching and mentoring to care management staff. This involves observing their performance, identifying areas for improvement, and providing constructive feedback and support. The educator will work closely with individuals to help them develop their skills and enhance their professional growth.
4. New Coworker Orientation: The Clinical Educator will also be responsible for conducting orientation sessions for new coworkers joining the care management department. These orientation sessions will provide an overview of department policies, procedures, and expectations. The educator will ensure that new coworkers are equipped with the necessary knowledge and resources to succeed in their roles.
5. Quality Improvement Activities: The Clinical Educator will actively participate in quality improvement activities within the care management department. This may involve analyzing data, identifying areas for improvement, performing audits and implementing strategies to enhance the quality of care provided. The educator will collaborate with the care management leadership team and other stakeholders to drive continuous improvement efforts.
6. Evaluate Programs: The Clinical Educator is responsible for evaluating the effectiveness of education programs. This includes collecting feedback from participants, analyzing outcomes, and making recommendations for program improvement. The educator will collaborate with the care management leadership team to ensure that training programs are meeting the needs of the staff and aligning with organizational goals.
7. Collaborate: The Clinical Educator will collaborate with other educators, clinical leaders, and subject matter experts to develop comprehensive and effective education programs. This may involve participating in interdisciplinary meetings, conducting research, and staying informed about industry trends and standards.
8. Record Training Participation: The Clinical Educator will maintain accurate and up-to-date records of training participation for care management staff. This includes tracking attendance, documenting completion of required trainings, and generating reports as needed.
Education Requirements:
● Bachelor's degree in a healthcare or business-related field. Master's degree preferred.
Licensure/Certification Requirements:
● Valid license as a Licensed Clinical Social Worker (LCSW) or Registered Nurse (RN) in North Carolina

● Minimum of 2 years of experience in Case Management

- Familiarity with adult learning theories and instructional design principles
