JOB SUMMARY

The MDS Coordinator position is responsible for assuring the timely completion of accurate assessments and interdisciplinary care plans for elders that meet Federal and State guidelines. Included are identifying resident acuity and needs, helping to determine specific care needs and communicating needs/expectations to families and responsible parties. The MDS Coordinator will also serve as the facility’s Infection Preventionist and assist the Director of Nursing (DON) with various tasks as defined below.



ESSENTIAL JOB FUNCTIONS


  • Completes assessments, Minimum Data Set (MDS) and care plans for all Elders.
  • Monitors completion of MDSs by other disciplines within time frames prescribed by regulatory guidelines.
  • Advises interdisciplinary team members of incomplete or untimely assessments by disciplines other than nursing.
  • Ensures accurate, timely completion of the MDS/RAPs/Triggers sheet for assigned residents.
  • Initiates, monitors and updates care plans and supporting activities that will result in the best possible outcome for each elder.
  • Generates and distributes a monthly care plan calendar for the following month.
  • Conducts care plan conferences for each elder and the interdisciplinary team (IDT).
  • Maintains and updates all care plans and assessments on a quarterly and as needed basis based on change in conditions and other pertinent issues.
  • Review all elder incidents and accidents and the 24-hour report daily to ensure that care plans for assigned residents reflect current and changing needs.
  • At patient care conferences facilitate and coordinate the activities of the disciplines of nursing, rehabilitation, dietary, activities, social services and restorative nursing.
  • Monitors and ensures completion of 7-day look-back documentation by nursing staff within prescribed MDS time frame.
  • Participate in facility management meetings as requested.
  • Monitors audit clinical record documentation on assigned residents to ensure accuracy and timeliness.
  • Advises DON of persistent issues related to non-compliant documentation on assigned residents.
  • Promotes and participates in ongoing education, inside and outside the facility, to maintain and enhance knowledge of current / new regulations.
  • Maintains current knowledge of RAI Manual and MDS assessment process.
  • Attends in-service training and other educational programs as directed or authorized.
  • Performs all job duties to ensure that confidential information and resident rights are always protected.
  • Supports quality of cares and operational efficiency by maintaining attendance records.
  • Performs job duties IAW established safety guidelines, protocols and expectations.
  • Reports unsafe conditions and accidents to supervisor or another designated on-duty manager.
  • Supports and promotes facility and company standards for superior customer service by exhibiting positive, courteous, and helpful behavior when dealing with all internal and external customers.
  • Complies with established standards described in facility policies and procedures, code of conduct, corporate compliance plan, employee handbook and other company documents and publications
  • Along with the DON:
  • Functions as a liaison between the Elders, their families, staff and the general public - responding to inquiries and providing information on nursing care and services provided by GHLS.
  • Monitors the Restorative Care Program to provide restorative nursing that maximizes each elder’s abilities.
  • Communicates updates on a regular basis to the Clinical Support Team (CST).
  • Accompanies physicians and their assistants during their monthly rounds.
  • Provides leadership and mentorship to each cottage’s care teams.
  • Lead and participates as assigned in required programs, including, but not limited to, QAPI, Safety, Infection Prevention, Significant Change, Retention, Business Development, daily nurse’s meeting, administration meeting, etc.
  • Makes recommendations as to the purchase of medical equipment.
  • Assists in preparation of the federally mandated Facility Assessment.
  • Stays abreast of changes in regulations and nursing practices, and changes to long-term care environment.
  • Regularly visits each Green House Cottage to monitor clinical cares and individual status of the Elders.
  • Monitors timely compliance of collection and submission of MDS data per organizational policies.
  • Works with the DON and Interdisciplinary Team (IDT) to identify areas of quality improvement.
  • Attends self-managed work team meetings as necessary to communicate nursing issues/concerns. Provides input for team discussion and seeks feedback from the team.
  • Upholds the Green House Project values of real home, meaningful lives and empowered staff while simultaneously ensuring compliance with state and federal regulations.
  • Works cooperatively and communicates effectively with the administrative team and family members to ensure ongoing patient-centered care.
  • Leads and assists in the development of the care teams and assumes leadership presence in the absence of and as delegated by the DON. This includes coordination of the care team schedule and on-call duties on a regular rotation.
  • Will work at minimum one shift per work performing direct care up to the scope of the nurse. Shifts will rotate throughout the campus to assist in familiarity with all elder’s needs and level of care.

POSITION QUALIFICATIONS

Education / Experience / License and Certifications


  • Bachelor’s degree in nursing, required.
  • Current, unrestricted Registered Nurse license.
  • Minimum two years’ experience in a supervisory or management capacity.
  • Minimum of one year of skilled nursing experience, required.
  • Experience with MDS 3.0, preferred.
  • Experience with Infection Control.
  • BLS, required.
  • Central Registry, required.

  • Able to communicate effectively in English, both verbally and in writing.
  • Additional languages preferred
  • Strong written and verbal skills
  • Basic computer knowledge

Specific demands not listed: Possible exposure to blood and or body fluids / infectious disease / hazardous waste requiring the use of Personal Protective Equipment. Exposure to odorous chemicals / specimens and Latex products.
