Position Summary / Career Interest: The Integrated Nursing Case Manager, under the direction of the Director/Manager of Case Management, provides care/service safely and efficiently for a full range of services to patients of all ages and their families. Primary role is to collaborate, communicate and facilitate coordination of services during hospitalization and post-hospitalization as established by the healthcare team. Integrated Nursing Case Manager concurrently facilitates proactive patient care services to ensure that care is appropriate, timely, and cost-effective and achieves the desired outcome. The Integrated Nursing Case Manager coordinates the care and service of selected patient populations across the continuum of illness; promotes effective utilization and monitoring of health care resources; and guides all disciplines toward positive quality outcomes. Responsibilities: Collects and assesses initial and on-going data about the health status of the patient. Initiates and/or revises an individualized plan of care that includes immediate and long-term outcomes for the patient/family and reflects the plan of the entire health care team. Carries out interventions safely and in compliance with policy to facilitate achievement of expected outcomes. Evaluates patient response to intervention/therapy based upon expected outcomes. Provides patient education by explaining procedures, medications and giving instructions at a level the patient can understand; addresses patient’s questions and concerns; verifies patient informed consent when indicated. Provides patient, family and staff education regarding wound management, prevention of pressure ulcers, support surfaces, dressing selections and other therapies. Documents and communicates all required components of patient care. Coordinates treatment to provide comprehensive wound assessments and plan of care including goals, implementation of treatment, evaluation and outcomes for patients throughout the organization. JOB REQUIREMENTS Required: Registered Nurse in State of Kansas required Bachelor's Degree in Nursing required Minimum two (2) years clinical experience in health care setting preferred Minimum two (2) years clinical experience required Ability to proficiently read and document in electronic medical record required. Basic typing and word processing ability required.
