Job Details Location: Niles, MI Department: HomeCare Health MB - Manager Patient Care Services Status: Full time Shift: Day (United States of America) Hours: 8 a.m. to 4:30 p.m. with Occasional Weekends and Holidays Job Summary Provides skilled nursing care to clients in a home setting or facility. Must have ability to apply nursing principles and practices. • Make the initial evaluation visits and regularly re-evaluates and assists the patient and caregiver in meeting related nursing needs. • Initiate the plan of care and care plan, makes necessary revisions, furnishing those services requiring substantial and specialized nursing skill. • Initiate appropriate preventative and rehabilitative nursing procedures. • Initiate appropriate chart documentation and able to prepare clinical and progress notes. • Submit a written clinical report following each visit including observation of patient’s condition and a progress report. This report will be incorporated into the patient’s clinical record. • Participate with other health personnel of the agency in planning for the implementation of the Plan of Treatment and discussions for the purpose of planning and evaluating patient care in individual cases. • May participate in medication, special needs services and/or foot clinics. • Give skilled care to patients requiring professional nursing services. • Committed to the continuous improvement of the patient experience which includes patient safety, quality of care and excellent service. Requirements Education: Certifications: *LPN License - State of Michigan Skills: Additional Job Details Physical Requirements: Continuously: Standing, Walking, Lifting/exerting force: 0-10 lbs., Use of hands to grasp/handle/feel, Vision, Vision-color, Hearing, Speaking Frequently: Lifting/exerting force: 11-25 lbs., Lifting/exerting force: 26-51 lbs., Bending, Squatting Occasionally: Sitting, Stoop/kneel/crouch/crawl, Climb/balance, Operating a motor vehicle • Skills: Basic computer, data entry, typing, filing and organization skills required. Must have effective communication skills. Will spend the majority of work time in conversation with patients and other healthcare members. Must be able to work in a self-directed and collaborative environment. Must exhibit strong organizational, documentation and teaching skills. • Abilities: Ability to work extended hours beyond normal work schedule to include, but not limited to, evenings, weekends, extended shifts and/or extra shifts, various locations, sometimes on short notice. • Environmental Conditions: Potential exposure to fumes, dust, gases, and odors. Frequent changes in temperature may occur. Possibility of exposure to communicable diseases.
